Physician Orders for Life-Sustaining Treatment (POLST)

Physician Orders for Life-Sustaining Treatment (POLST) compliment an Advance Directive rather than replace them.  A POLST is a physician order sheet based upon the patient’s current medical condition, and more important, the patient’s wishes.  If any one section in the POLST, physicians will render full treatment for the incomplete section.  Topics within POLST include cardiopulmonary resuscitation, medical interventions, artificially administered nutrition, as well as information and nutrition.  California recently updated its POLST form in October 2014 (in multiple languages, including English, Spanish and Russian, among others), links for which can be found here and here.
